Please mail a hard copy of your application AND e-mail a file to:

Please submit your request along with a cover letter signed by the chief professional or chief volunteer officer of your organization. Attach the following required documents:
- IRS 501(c)(3) letter of determination
- Project Budget (including a list of other sources of funding)
- Copy of most recent IRS Form 990 and independent audit
IMPORTANT: Your proposal will not be eligible if:
- The request is for other than capital needs (e.g. building construction or repair, equipment and supplies).
- You have received a grant from the Carlilian Foundation within five years of the date of your proposal.
